On this and the following pages you will be
asked to provide your personal data for filing
your official application for a US green card.
Please fill in the empty spaces and click
on the “Next>”-Button to go
to the next page. After completing the last
page, a new window will open to display the
official entry form. Please print this form
using your browser’s “Print”-Button,
sign the form where it is indicated, attach
a recent passport picture and send us the
application form by mail.
Attention: You
will have to submit one photo each of yourself
and all family members that you list on
your application (i.e. a separate photo
each of yourself, your spouse and all unmarried
children under the age of 21). All passport
photos should be between 37mm x 37mm and
50mm x 50 mm (1½–2 inches) square
in size, with the name and date of birth
of the person shown on the photo printed
on the back. Please take the pictures directly
facing the camera in front of light-colored
background.
It is very important that
you send one photo
of each family member!
